View Your Medical Records

In line with government guidelines Handbridge Medical Centre have enabled patients to view their medical records as part of Patient Access.

Being able to see your record online might help you to manage your medical conditions. It also means that you can access it from anywhere in the world should you require medical treatment on holiday.

To apply for access to your medical records, please print and complete the following online form or request a questionnaire from reception next time you are at the practice.
