If you have symptoms associated with coronavirus including a new continuous cough and a high temperature, you are advised to stay at home for 7 days.


Do not book a GP appointment or attend your GP Practice.


If your symptoms are serious, or get worse, NHS 111 has an online coronavirus service that can tell you if you need further medical help and advise you what to do.

Online 'Pre-registration' with the practice

PreRegistration

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Submit" button to mail your form to us.

In order for your registration to be accepted by the Practice, you will need to attend in person in order to sign the registration form.  You will need to bring with you proof of address and ID.  If you have not already done so we will ask you to complete a health questionnaire (see link below).*

*During the COVID-19 pandemic if you are unable to attend surgery to provide POI & POA this can be scanned/photographed and sent to handbridge.medicalcentre@nhs.net to complete your registration. We also will need a scanned/photographed patient contract available to download on the website. PLEASE NOTE THAT YOU MAY BE ASKED TO VERIFY THIS AGAIN IN PERSON ONCE THE PANDEMIC HAS SETTLED.

Please note all forms that are not signed/been provided with accompanying proof of ID/Address are discarded after 1 month.

It can take a considerable amount of time for your medical records to come through to us. Therefore we ask that all our new patients complete a Medical Questionnaire so that we may offer you a 'New patient appointment' where appropriate. 

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
