If you have symptoms associated with coronavirus including a new continuous cough and a high temperature, you are advised to stay at home for 10 days.


Do not book a GP appointment or attend your GP Practice.

Online Registration with the practice.


PreRegistration


You may register at Handbridge Medical Centre as a new patient if your postcode begins with one of the following as you are in our practice area;


CH1 1, CH1 2, CH4 7 & CH4 8


If your postcode does not begin with any of these, then unfortunately, at this time we would be unable to register you as you are outside of our practice area, please click here to take a look at NHS choices for alternative practices who may register you. Please keep in mind your nearest practice may be in Wales, for Welsh surgeries please click here.


The easiest way to register at the practice is to attend in person to collect a registration pack and return it to us in person with proof of address and identity so we are able to verify your documents.


If you are unable to attend the practice, all the forms required are available to download below; please submit any documents in one single email only.


Please note in order to verify any identity/address documents you have emailed in we will need you to attend practice when you are next able to.


Please also note; we must receive all 5 items detailed below; any forms received where the other accompanying items have not been received will be discarded after 4 weeks.

Please ensure all the below documents are received, your registration will not be processed until we have:


1. New Patient Registration form


(Click above to download) This must be printed and completed, ensure you provide a signature and you MUST provide your NHS number. If you do not have this, you can contact your previous surgery to obtain it. You can then hand-deliver this to the practice or scan/photograph a copy over to handbridge.medicalcentre@nhs.net (please ensure any other documents needed to register as detailed in this list are sent in the SAME single email). 


2. Online medical questionnaire for new patients


(Click above to submit the form online) The medical questionnaire can be completed on the above link and submitted electronically.


3. Patient Contract


(Click above to download) This must be printed and completed, ensure you provide a signature. You can then hand-deliver this to the practice or scan/photograph a copy over to handbridge.medicalcentre@nhs.net (please ensure any other documents needed to register as detailed in this list are sent in the SAME single email). 


4. Sharing Consent


(Click above to download)  This must be printed and completed, ensure you provide a signature. You can then hand-deliver this to the practice or scan/photograph a copy over to handbridge.medicalcentre@nhs.net (please ensure any other documents needed to register as detailed in this list are sent in the SAME single email). 


5. Proof of address & Proof of ID


Both of these items must be verified by the practice. We can accept a driving licence/ passport for proof of ID. Bank statement, utility bill, rental contract, mortgage letter, (from the last 3 months) for proof of address.


 You can then hand-deliver these to the practice to be verified or scan/photograph a copy over to handbridge.medicalcentre@nhs.net (please ensure any other documents needed to register as detailed in this list are sent in the SAME single email).


ID documents received via email may be asked to be re-verified in person at a later date.
